We will collect information about your shopping habits, your perception of membership / dividend card. If you are interested in the prize draw, we will collect your name, email address and phone number.

The Survey is operated by Habit 5 Limited, based in the United Kingdom and the details you share with us as part of this survey is shared and registered with them. This Privacy Notice for Survey Respondents should be read in conjunction with the Data Privacy and Policy of Lincolnshire Co-operative Limited (“Lincolnshire Co-op” or “the Society”) available on our website at Privacy policy (lincolnshire.coop).

We assure you that data collected in this survey will be secured against unlawful and unauthorized access. Where necessary, the published results and findings will be anonymized.

You are entitled to the following rights:

• request for and access personal data collected and stored by Lincolnshire Co-op;

• request rectification and modification of data kept by Lincolnshire Co-op;
• request for deletion of data;

• You must be a resident of the UK and 18 years of age or over to be eligible.

• Employees of Lincolnshire Co-op are eligible to participate, except those directly involved with the prize draw administration.

• You will only be entered into the Prize Draw once.

• Use of a false name or address in the Research will result in ineligibility for the Prize Draw.

• There will one prize of £100 cashback paid on to the dividend card of the winner. Plus a £100 donation made by Lincolnshire Co-op to a charity of the winner's choice.

• The Prize cannot be modified and is non-transferable.

• The Prize Draw selection is final and no correspondence will be entered into.

• One winner will be selected at random in the Prize Draw.

• Only the winner will be contacted and notified by email, no later than 7 days from closure date shown above, or such later date within Lincolnshire Co-op’s discretion.

• Reasonable efforts will be made to contact the winner. If the winner cannot be contacted, or is unable to comply with these Prize Draw Terms and Conditions, Lincolnshire Co-op reserves the right to offer the Prize to an alternative entry

• Failure to respond, or failure to meet the eligibility requirements, may result in forfeiture of the Prize.

• By participating in the Research, you agree to grant Lincolnshire Co-op a perpetual, royalty-free, non-exclusive, sub-licensable right and world-wide license to use, reproduce, modify, adapt, publish, translate, create derivative works from, distribute, perform, display and exercise all copyright and publicity rights with respect to any non-identifying Research data, and/or to incorporate data in other works, in any media now known or later developed for the full term of any rights that may exist in your data, and in accordance with privacy restrictions set out in Lincolnshire Co-op’s Privacy Policy.

• Lincolnshire Co-op may retain anonymised Research data results indefinitely and may choose to incorporate it on any of its websites in the future. Any future publication of non-identifying results or sharing of the results with its partners will be at Lincolnshire Co-op’s sole and absolute discretion.

• Lincolnshire Co-op takes privacy very seriously, and we will only use your personal information in accordance with the terms of our Privacy Policy. By participating in the Research you acknowledge and agree that you have read and accepted the terms of our Privacy Policy and these Terms and Conditions.

• Lincolnshire Co-op excludes all liability under these terms, in contract or tort (including negligence) for any loss, damage, costs and/or expenses, loss of profits, goodwill, consequential, indirect or special losses, howsoever caused, suffered or incurred in connection with the Research and the Prize Draw.

• We reserve the right to vary these Terms and Conditions at any time. Any variations will be notified to you by email and/or by posting the variations to our website.

• These Terms and Conditions (as amended from time to time) constitute the entire agreement between you and Lincolnshire Co-op concerning your participation in the Research and Prize Draw.
